“Kōji” Not the Culprit: Finding the Causes of the Kobayashi Pharmaceutical Supplement Scare

Five are dead and dozens more hospitalized for kidney damage after taking dietary supplements from Kobayashi Pharmaceutical. All eyes are on the beni kōji red yeast rice contained in them, but a fermented foods specialist warns against blaming this ingredient, or kōji mold more broadly, without understanding the science.

Major Health Scare, Misplaced Fears

Japan is today in the midst of a nutrition-related scare, with dozens hospitalized for kidney damage and multiple deaths reported in connection with dietary supplements produced by Kobayashi Pharmaceutical. This scare is spreading rapidly through society in part because of a lack of understanding of the main ingredient in the products in question, beni kōji, or red yeast rice.

As a researcher specializing in fermented foods, it concerns me greatly to see the commonly held misconceptions about kōji, the Aspergillus oryzae mold used in the production of everything from soy sauce and miso to sake and shōchū, and beni kōji, which—despite sharing the kōji character in its name with the former—is a different substance altogether, the product of strains of the Monascus yeast species grown on rice. It needs to be stated clearly that beni kōji is unlikely to be at fault in the ongoing crisis, and kōji is a broadly used substance that poses no threat whatsoever to health.

As far back as the Kamakura (1185–1333) and Muromachi (1333–1568) periods, Japan was home to a thriving industry making use of kōji in foodstuffs and beverages. High-quality kōji mold was cultivated by seed kōji growers known as moyashiya and sold to sake breweries and other businesses.

Beni kōji, meanwhile, was traditionally used on the Chinese mainland and in Taiwan to produce drinks like hongjiu and laojiu, “red liquor” and “aged liquor,” respectively. It is also an ingredient in the traditional Okinawan dish tōfu yō, a fermented tofu with a consistency like cheese. In the production of all these drinks and foods, beni kōji has a lengthy history and its safety as an ingredient has long been recognized.

The Need to Nail Down the Unknowns

The dietary supplements suspected of causing health damage to users contain beni kōji and promise to lower cholesterol. The particular products in question were made at a limited number of factories during a specific period of time, making it less likely that the harm is due to the basic ingredients in them. Kobayashi Pharmaceutical has announced the possibility that an unknown substance was introduced to them at some point during the manufacturing process. Needless to say, the media’s reporting on a dangerous “unknown substance” has prompted fear among many.

I must stress, though, that it is only natural for unidentified substances to be included in products made through fermentation. The world of microorganisms is one where we cannot be certain what reactions will take place, and to what extent, due to variations in the condition of those microorganisms and the environments where they grow. This is a field where we still have much to learn, and research in the area is constantly turning up new discoveries.

Of course, we never want to see the inclusion in food, drink, or supplements of unknown substances that pose a threat to health. To prevent this, humans have put centuries of experience and scientific knowledge to use in selecting the microorganisms that pose the least risk. Until the current scandal emerged, Kobayashi had also been controlling this risk appropriately, with strict management of its production processes.

When the first reports emerged of kidney damage, suspicion fell on a contaminant called citrinin. This is a form of mold that may be produced as a byproduct of beni kōji in some cases, and scientists have identified it as a threat to kidney health. The European Union has pointed to citrinin as a reason to heavily regulate beni kōji supplements; in some countries, all sales of products containing beni kōji are banned.

Kobayashi, though, produces all of its own beni kōji in house, and genomic analysis has confirmed that the seed strain it uses does not produce citrinin. Analysis of the content of the supplements used by people suffering kidney damage has also shown that citrinin is not present in them. This leaves the possibility that despite the company’s mechanisms to control its ingredients and production processes, another unexpected substance has been formed in or introduced to the products at some stage.

Until we can identify that substance, though, consumers will continue to be suspicious of these products. Over a week has now passed since the scandal came to light, and we are starting to hear theories about contaminants making their way into the supplements during production, or sudden mutations in the beni kōji yeast being used. We can only hope that the true cause will be pinpointed as soon as possible.

Serbian Twists:

  1. Sit on the floor with your knees bent and feet lifted a few inches off the ground.
  2. Lean back slightly, keeping your spine straight and core engaged.
  3. Clasp your hands together in front of you or hold a weight for added resistance.
  4. Twist your torso to the right, bringing your hands or weight beside your hip.
  5. Return to the center and then twist to the left.
  6. Continue alternating sides for the desired number of repetitions.
    This exercise targets the oblique’s and helps strengthen the core muscles. Remember to maintain proper form and control throughout the movement.

Small tea growers suffer after purchase of green leaf stops

Guwahati: Small tea growers in the state are suffering due to a sudden decision by bought-leaf factories to stop the purchase of non-certified green leaf  from today. Amidst the prevailing situation, the growers also stopped plucking tea leaves in some districts as the factories almost came to a standstill.

A few days ago,  leaf factories decided not to procure green tea leaves from growers if they did not provide laboratory testing reports, certifying that no banned chemicals or unapproved pesticides were used in growing their tea bushes. They stated that their decision would be applicable starting today (April 1).

Following this, an undesirable situation prevailed among the small tea growers, as the tea season had just started. The decision by the bought leaf factories also led to a scramble among the growers as the only laboratory for testing tea leaves exists at the Tocklai Tea Research Institute in Jorhat. It was considered an impossible task for tea growers from across the state to bring their produce to Jorhat.

The situation came to such a point that all bought leaf factories stopped buying green leaf, and the growers also decided to suspend plucking operations as there were no buyers for their green leaf.

Talking to The Sentinel, secretary of the Jorhat district committee of All Assam Small Tea Growers Association, Krishna Prasad Sharma, said, “From today, bought leaf factories suddenly stopped purchasing green leaf without a lab-testing report. Tea leaf testing facilities only exist at Tocklai, and their capacity is also limited. Moreover, it is impractical and not feasible for small tea growers from other parts of the state to bring samples to Jorhat for testing. Today, we met with officials from Tea Board of India, Jorhat. They informed us that there were no instructions issued by them for mandatory testing and certification of green leaf. The Board only issued directions for the compulsory auction of 100% dust-grade tea, initially for three months. So, we submitted a memorandum to the Board, asking them to resolve the situation. Another memorandum was submitted before the Jorhat DC to ease our suffering. We request the Tea Board and the government to resolve the issue as early as possible.”

Sharma also informed us that they have stopped plucking green leaves in Jorhat district for a period of seven days from today, in view of the current situation.

Meanwhile, the North Eastern Tea Association (NETA) wrote to the Golaghat DC, which read, “We commit to abide by the FSSAI norms and will not accept any green leaves without accompanying test reports from a NABL-accredited lab, effective April 1, 2024. The purpose of testing green tea leaves is not to know MRLs in the leaf but to know if any ‘Detectable Residues of Pesticides’ are present in the green leaf or not. The purpose of testing green tea leaves is to ensure that green tea leaves are free from banned and non-approved pesticides that may later be found in Made Tea.”

As lakhs of local youth are involved in the small tea industry, an early resolution of the prevailing situation is being desired by people concerned about its development.

Our Golaghat correspondent adds: Keen reactions were observed over not purchasing tea leaf from small tea growers by the bought leaf factories in Golaghat district.

In this regard, the All Tai Ahom Students’ Union (ATASU) of Golaghat district unit submitted a memorandum to the District Commissioner (DC) on Monday regarding the purchase and sale of green leaf of small tea growers. The bought leaf factories made the decision to purchase tea leaves only with a banned pesticide-free certificate from the small tea growers. Otherwise, the factories refused to accept the green leaf from tea growers from April 1. It was not possible for all small tea growers to get banned pesticide-free certificate from the concerned authority in such a short time. Most of the small tea growers depend on the green leaf business for their day-to-day lives. In this situation, if the bought leaf factories did not purchase the green leaf tea growers, they would face hardship in running and maintaining their families. In the memorandum, the ATASU president in charge, Pankaj Gogoi, and secretary Dipjyoti Tamuli demanded that the District Commissioner look into the matter and take initiative to continue the process of selling and purchasing the green leaf as usual without hampering the small tea growers. Further, they also demanded a stop to the sale of banned pesticides in the market.

Best Form of Vitamins for Absorption

Vitamin absorption depends on many complex factors, including form, diet, and health. The best form of vitamin supplement for you depends on your health and needs.

Capsules, tablets, liquids, and powders — many different types of supplemental vitamins are available today.

Researchers have long begun studying how different nutrients are absorbed and used in the body.

Although more research is needed, scientists believe a variety of factors affect nutrient absorption, including dietary fat intake, interactions between foods and vitamins, and even stress.

If you’re looking to get the most out of vitamin supplements, there are several ways to improve absorption.

This article reviews everything you need to know about which vitamin supplements offer the best absorption and how to maximize vitamin absorption.

Vitamin supplements are available in a variety of forms, including gummies, pills, liquids, and powders.

Research, such as this 2022 study, has looked into whether liquid supplements absorb more easily than forms like powder or tablets. However, more studies are needed to support this claim.

A 1982 study on vitamin C supplements suggests they absorb equally well in powder, chewable tablet, and non-chewable tablet forms. However, the absorption of vitamin C supplements in slow release forms is unclear.

Although the bioavailability — meaning the ability of a nutrient to be absorbed and used by the body — of some nutrients are fairly well understood, research into the uptake, absorption, and bioavailability of other nutrients in humans is still at an early stage.

Some vitamin manufacturers add coatings to pills to enhance flavor or shelf life. A 2019 article on vitamin D supplements found that coatings may affect the dissolution or absorption of a vitamin.

Water-soluble vs. fat-soluble vitamins

In addition to a vitamin’s form, many other factors affect absorption. For example, water-soluble vitamins absorb differently than fat-soluble vitamins, and others are stored in the body.

There are two main types of vitamins: water-soluble (all eight B vitamins and vitamin C) and fat-soluble (vitamins A, D, E, and K).

Water-soluble vitamins dissolve in water and are generally not stored in the body. Excess amounts exit the body through urine.

These vitamins must be consumed regularly to prevent shortages or deficiencies in the body. However, one exception is vitamin B12, which can be stored for years in the liver.

Fat-soluble vitamins dissolve in fats, which then travel through the small intestines and are eventually distributed through the bloodstream. Excess amounts of these vitamins are stored in the body’s liver, fatty tissue, and muscles for later use.

According to a 2021 review, other factors that can affect the absorption of nutrients include:

  • interactions with other foods or vitamins
  • dietary fat intake
  • alcohol
  • age

However, the review studied the absorption of food sources of vitamins, not vitamin supplements. More research is needed on the absorption of vitamin supplements to understand which forms absorb more easily than others.

Here are some steps you can take to improve vitamin absorption.

Pair complementary nutrients

Certain nutrients help each other absorb. For example, fat-soluble vitamins are better absorbed when consumed with a small amount of healthy fat, such as olive oil, avocado oil, or almond butter.

Additionally, plant-based sources of iron, such as beans, lentils, and nuts, require vitamin C for absorption. Pairing them with vitamin C-rich foods or vitamin C supplements may improve absorption.

Maintain a healthy gut

Your gut plays an important role in the absorption of nutrients from the food you eat. Maintaining a healthy gut may help make sure your body absorbs essential vitamins and minerals.

Reduce stress

Research suggests chronic stress may increase the body’s metabolic needs as well as the use and excretion of nutrients. This may lead to low micronutrient levels in the body.

  • getting regular exercise
  • having good sleep hygiene
  • practicing mediation or deep breathing
  • signing up for mental health counseling

Consider reducing or avoiding alcohol

Alcohol consumption may interfere with the absorption of nutrients, including water-soluble and fat-soluble vitamins. Frequent alcohol use can also lead to malnutrition.

Consider limiting or avoiding alcohol consumption to make sure your body properly absorbs essential nutrients.

What vitamin supplements are good for absorption?

The best form of vitamin supplements for you depends on your individual needs. For example, if you have difficulty swallowing capsules or tablets, you may prefer a liquid vitamin.

Which vitamin is hardest to absorb?

Vitamin B12 requires the help of a protein called intrinsic factor for normal absorption. Some people lack this protein and may need injections or high dose supplements of vitamin B12.

Other common vitamin and mineral deficiencies include vitamin B6, iron, vitamin D, and vitamin C. Factors that are associated with these deficiencies include certain health conditions, diet, pregnancy, race, and ethnicity.

What two vitamins should not be taken together?

Long-term consumption of excessive zinc can lead to copper deficiency. To prevent copper deficiency, the U.S. Food and Nutrition Board recommends a tolerable upper level intake of 40 milligrams per day of dietary and supplemental zinc for adults.

According to the National Institutes of Health (NIH), some reports suggest high intake of vitamin C may reduce vitamin B12 and copper levels. However, additional studies have not supported these conclusions.

What is the most bioavailable form of vitamin A supplement?

Vitamin A supplements are available in two main forms: preformed vitamin A (found in foods from animal sources such as meat, fish, and dairy products) and provitamin A carotenoids (found in plant products such as green leafy vegetables).

According to the NIH, the most bioavailable form is preformed vitamin A. The absorption of preformed vitamin A supplements ranges from 70–90%, while the absorption of provitamin A beta carotene ranges from 8.7–65%.

Although researchers are studying the absorption and bioavailability of nutrients, more research is needed to understand the absorption of vitamin supplements in various forms.

Many online reports claim that certain forms of vitamins absorb more easily than others. But a broad consensus in the scientific community is lacking.

Some research suggests pairing vitamins, maintaining good gut health, and avoiding alcohol may improve absorption of nutrients. However, this research has focused on food sources of vitamins rather than vitamin supplements.

The best form or type of vitamin for you depends on your individual needs and preferences. Most people can get all the nutrients they need by eating a healthy, balanced diet.

It’s not normal for people to experience pain during sexual intercourse or penetration. Yet, about 3 out of 4 people assigned female at birth report experiencing painful sex, called dyspareunia. Experiencing pain during sex is not only uncomfortable, but affects your pleasure.

For people with female anatomy, painful sex may indicate an underlying condition that needs to be addressed. One group of medical conditions known to cause painful sex is chronic gynecologic conditions, like endometriosis and polycystic ovary syndrome (PCOS).

But even if you’re living with a gynecologic condition, you don’t have to put up with painful sex. Treatment and open communication with your partner can lead to better experiences.

Here’s a look at chronic uterine conditions and how they can influence your sex life.

What are chronic uterine conditions?

Chronic uterine conditions, also called uterine diseases, are any conditions that affect your uterus. And they can be caused by hormonal imbalances, thyroid problems, polyps, cancer, infection or pregnancy.

“There are multiple uterine conditions that impact women,” said Barb DePree, M.D., MSCP, an OB-GYN and a member of HealthyWomen’s Women’s Health Advisory Council. She mentioned that the more common uterine conditions include uterine fibroids, adenomyosis, pelvic organ prolapse and pelvic inflammatory disease.

These conditions can be treated with birth control pills, GnRH antagonists, pain medicine or — in some cases — surgery.

Chronic uterine conditions can impact your life

A common complaint that people with chronic uterine conditions have is persistent pelvic pain that lasts six months or more.

Symptoms of chronic pelvic pain include:

  • Severe and steady pain
  • Pain that comes and goes
  • Dull aches
  • Sharp pains or cramps
  • Pressure or heaviness in the pelvis

This pain can happen during sexual intercourse or penetration, when you pee or poop, or when you stand for extended periods of time.

How do chronic uterine conditions affect your sex life?

Experiencing pain during sex can keep you from experiencing sexual pleasure. And that, in turn, can also contribute to having a low sex drive. The burden of dealing with the physical symptoms of uterine conditions can also cause mental health issues like depression and fatigue, which also can impact your desire to have sex.

According to DePree, how you view your condition also plays a big role in how you approach sex. “Some women don’t allow that to interfere in their enjoyment of intimacy, and [for] other women, it might be a distraction or difficulty for them,” she said.

How can partners work through potential pain and bleeding?

You shouldn’t have to navigate painful sex or bleeding without the help of your partner. Communication is important to having great sexual experiences if you’re managing a chronic uterine condition.

Should I take a multivitamin? Here’s what the science says.

Q: What are the benefits of taking a multivitamin? Should I take one even if I’m healthy?

A: About one-third of Americans take multivitamins regularly. Many purchase the supplement because they think it can impact broad health outcomes — help them live longer, decrease cancer risk and reduce cardiovascular disease. Otherwise healthy people also take it to fill gaps in their diets.

I wish it was that straightforward. Though there’s some encouraging data on the benefits of multivitamins for healthy adults, the supplement isn’t a cure-all.

There have been three main studies looking into whether multivitamins impact important health outcomes, all of which had private and public support, including from supplement companies:

  • A French study of about 13,000 adults between ages 35 and 60 found an improvement in cancer risk and mortality in men, but not women, after 7½ years of daily supplementation.
  • Another study investigated over 14,000 U.S. male physicians age 50 or older and showed that multivitamin use for a median of about 11 years was associated with an 8 percent reduction in cancer, though there was no difference in mortality.
  • A study called COSMOS examining multivitamin use over a median of about 3½ years in over 21,000 older adults found no improvement for men or women in cancer risk, mortality or cardiovascular disease. An ancillary study did find that certain aspects of cognition significantly improved for men and women over age 60.

Taking a multivitamin daily is generally regarded as safe. But because there isn’t enough evidence of benefits for healthy adults, the United States Preventive Services Task Force does not recommend it.

My takeaway? Eating a healthy diet has been well established in multiple large studies to improve numerous health outcomes, from reversing coronary heart disease to reducing cancer risk.

You can’t redeem a poor or unbalanced diet by popping a pill to fill in the nutrient gaps.

Is it better to eat healthy or take vitamins?

Many people have studied the health benefits of supplementing particular nutrient gaps with a pill — for example, magnesium, which is popular as a supplement and a common feature of many multivitamins.

Studies consistently find that magnesium supplementation is insufficient to achieve the benefits that come with a diet high in magnesium. This is likely because of numerous other health benefits that accompany a balanced diet that are not easily distilled to one — or even a handful — of nutrients taken out of their original forms.

Do multivitamins help cognition?

A large study published in 2023 derived from the COSMOS database found that people taking a multivitamin for a year performed better on an immediate word recall test. In this test, participants read 20 unique unrelated words several times in a row, then were asked to read a new list of 20 words as a distraction, before being asked to recall as many words as possible from the first list. The study authors concluded that multivitamin supplementation had the effect of improving age-related memory changes on this test by about 3.1 years.

This data is encouraging. We likely need additional research to understand more about the extent of this effect and to target the right population. For instance, the people who appeared to benefit most are those with baseline cardiovascular disease. It’s not clear why, but it may be that these patients had micronutrient deficiencies, perhaps related to certain cardiac medications.

Still, studies that have used a more extreme endpoint, like the development of dementia, have not shown a benefit from over-the-counter supplements — something that is associated with a healthy diet. And in the 2023 study, multivitamins did not have a significant impact on other cognitive tests evaluating executive function or new object recognition.

Who should take a multivitamin?

There are a few scenarios where we know multivitamins are important:

  • People with nutritional deficits: This could include people with alcohol use disorder or elderly people living in long-term-care facilities who may have poor fruit and vegetable intake. Other people with specific nutritional deficits, such as vitamin B12 deficiency among vegans, could also reasonably take a multivitamin daily to target their deficiency if a multivitamin contains an appropriate percent daily value.
  • People who have undergone bariatric surgery. These patients need to be careful to ensure they are also meeting their calcium as well as iron, copper and zinc goals in their supplementation, as many multivitamins may not contain a full daily value of minerals.
  • People who are pregnant or breastfeeding. In this case, a multivitamin containing 400 to 800 mg of folic acid should be taken as directed by their physician.

How should I choose the best multivitamin for me?

Multivitamins are not regulated as medications by the Food and Drug Administration, so there can be wide variations in what goes into each brand’s formulations.

Typically, they contain some percent daily value of key vitamins and minerals. Some brands may contain 70 percent of the daily value of one such nutrient — say, vitamin C — while others contain 200 percent.

Do you really need 200 percent of the full daily value of vitamin C supplemented if you’re healthy? Few clinical trials exist to give these kinds of specific answers.

But generally, it makes most sense to pick a vitamin that targets your sex and age group. Multivitamins targeted at younger women tend to contain higher amounts of folic acid (in case of pregnancy) and iron, which is often low because of losses due to menstruation.

Putting anything into our bodies involves a risk-benefit calculation. Many people feel that taking a multivitamin is better than nothing and that the risks are extremely low. While I agree with at least the second part of that reasoning, in the bigger picture, the harm may come less from the multivitamin itself than from what we as health-care providers and society fail to address more fundamentally. When an otherwise healthy patient tells me about their multivitamin, I use that as a starting point to explore what nutrients they may be worried they’re not getting through their meals and ways we can support them to eat a more balanced diet.
