November is National Family Caregivers Month.
Award-winning actress Bellamy Young is known for playing strong characters on TV shows like “The Other Black Girl” and “Scandal” (hello, Ms. President!).
These days she’s also using the strength of her voice and personal experience to bring awareness to a serious condition called hepatic encephalopathy (HE) that can cause brain damage in people with liver problems. Young’s father was diagnosed with overt HE when she was a teenager.
“My dad’s journey with overt hepatic encephalopathy started with a cirrhosis diagnosis. That diagnosis came out of the fact that he was a big drinker,” she said. “The diagnosis came with a lot of shame and stigma — our family sort of took it on the chin and went home and silently agreed never to talk about it again.”
HE develops when your liver fails to filter toxins in your blood. The toxins then build up and damage the brain which can cause memory loss, slurred speech, coma and even death.
Young and her family had no idea that her father’s liver disease could cause HE and affect his brain. “We weren’t told much about anything we could do about it, and we definitely weren’t told what might be down the road for us or what to be on the lookout for,” Young said. Symptoms of HE can include involuntary movements, balance issues, a musty odor and dramatic shifts in personality — all things that happened to Young’s father. “When your brain is muddied by the toxins that are building up in your bloodstream and in your body, you turn into a different person,” she said.
Anyone with liver disease can develop HE, but you can stop it from causing permanent brain damage if you catch it early — something Young wishes she knew back then and why she’s so passionate about spreading awareness about HE today. We talked to Young about her experience and how she’s using social media to help give HE caregivers a voice.

HealthyWomen: Looking back, what signs of hepatic encephalopathy (HE) did you notice in your father?
Bellamy Young: We were living with this diagnosis [cirrhosis of the liver] that we thought was an end point, but my dad kept changing. I was a teenager and I was my worst self, and I just thought he’d been drinking during the day so that’s why he didn’t pick me up from school or that’s why he started having a funny smell. But then one night, he couldn’t find his way home from work — the building he worked at for 23 years — and that’s what got us back to the doctor because we thought something else entirely must be going on.
That’s when they said it’s overt hepatic encephalopathy and it’s the progression of his liver disease. My mom and I were like, “What?” Because we had no idea this was even a possibility. We felt terrible, like we could have helped him — we could have gotten him back sooner if we’d known that we were noticing changes. Options were very limited back then. We didn’t have much time with him after the diagnosis.
HealthyWomen: What inspired you to be open about your experience today and want to increase awareness about HE?
Bellamy Young: Learning that so much has changed. I had no idea because this isn’t something I hear talked about. I had no idea that treatment options have progressed and that communities are popping up — and once I found that out I was just so honored to be participating in the conversation.
Today, I’m so grateful to get to talk about it because I never want anyone else to feel ashamed or alone, and there’s so much more to know and there’s so much information and so many more options. And I’ve found talking about it — people are so relieved to have these conversations because they’ve felt so isolated and scared and just when we come together as a community and help each other — first and foremost — knowing we are not alone and then also sharing our knowledge and sharing and strategizing about how to talk with doctors and or how to care for our loved ones has been such a blessing.
HealthyWomen: Tell us a little bit about the HE caregiver conversations you’ll be sharing on your Instagram.
Bellamy Young: It’s simply sharing stories and the act of being seen and heard. The act of learning and coming together has been so powerful because — especially when you’re trying to care for someone you love — you so often forget to care about yourself too, and your well runs dry and there’s nothing left to give. And so, reminding ourselves that our greatest place of strength comes from a full well — or as close to as full well as we can get — and to ask for help when we need it and give help when we have the strength to without waiting to be asked.
HealthyWomen: What advice do you have or what you have learned that you want to share with HE caregivers?
Bellamy Young: A great thing to do is to make sure you know what to look for. If you know that someone you love has been diagnosed with any sort of liver disease, just know that it’s a journey — you’re not at your destination. Educate yourself about what other symptoms might be, what progression can look like, what might be on your horizon. HE might not happen, but just knowing what to look for will help you catch any changes as soon as they’re happening.
And then I really think the other pillar is community because you can’t do it alone. We always think we have to do it alone, especially if it’s our parents or our spouse or our child — we feel such a responsibility that’s only ours, but it takes a village to keep someone going and it also takes help to keep your own engine full of fuel. I’ve often had this conversation with myself — I have to think about self-care like it’s not icing on the cake — it’s a firm necessity.

We asked Mindy Caplan, an exercise physiologist certified with the American College of Sports Medicine, for her thoughts on what’s worth your time and when to stick to your routine.

What are vibration plates?: Remember in the ’80s when everyone was using a vibrating belt to shake away calories and lose weight? Well, the vibes are back — this time in the form of a small platform you stand on or use while you’re doing exercises such as pushups or squats.
Do vibration plates work?: The idea is that whole body vibration forces your muscles to contract and that can have health benefits such as weight loss and increased muscle strength. One review of six studies found that whole body vibration led to fat loss but not a real change in body fat percentage in studies longer than six months. Researchers noted the research is limited and more studies need to be done.
It’s also important to note that vibration may not be safe for everyone, including people who have a high risk of falling, and it may cause chronic issues in the nervous, musculoskeletal and circulatory system. As always, talk to your healthcare provider before trying a new workout device.
Caplan says: “I don’t know of any substitution for good old-fashioned resistance training to increase muscle mass, which is very important for longevity. On the flip side, if using a vibrating plate gives people the motivation to exercise more and eat better then by all means — use the plate — but use it in conjunction with weights and aerobic exercise. And hire a qualified fitness professional to meet those goals.”

What are walking pads?: Walking pads are mini treadmills that are just tread, rollers and motor — nothing fancy and nothing above the belt. The pared-down design has gotten a lot of attention (more than 113 million posts!) for being easier to use and store than a regular treadmill. Some models have a fold-up feature so check under your desk at home — you may have bought one during the pandemic and forgotten it’s there.
Walking pads are made for walking — not running — and have limited speed options. So, you’re not going to get a heart pounding workout on the pad, but research shows walking on a regular basis has a ton of health benefits, including stronger bones and muscles and lower risk for diabetes, high blood pressure and heart disease — the number one cause of death for women in the U.S.

Caplan says: “I love my walking pad! I got one for cold winter days when I can’t get out to walk. The only thing I can think of that might not work for some is the length of the belt. If someone has a long stride, the belt area can sometimes be too short and if it is too narrow the foot will hit the side of the unit and could cause a fall. Make sure to measure because the pads are much smaller than the treadmills at the gym.”

What is a weighted vest?: A weighted vest is just what it sounds like. It’s worn around the torso and adds resistance to any activity. (Note: Some styles veer more fishing vest than fashion depending on the look you’re going for.)
Fans of the vest say wearing one helps with weight loss — and that may actually be true. Caplan said wearing a weighted vest can help you burn more calories and increase strength.
However, weighted vests may not be a good option for everyone, including people with back pain and chronic pain. Caplan noted that the vests may cause joint problems and cause spinal compression and proper musculoskeletal alignment is crucial.
Caplan’s advice: “When considering a weighted vest, start light and gradually increase the weight.”

What is a mini trampoline workout? A mini trampoline routine involves bouncing on a small circular trampoline — also called a rebounder — as part of a full-body, low-impact workout. The exercise is particularly good for strengthening your glutes and research shows that it can improve balance and coordination among other health benefits.
Caplan says: “Mini trampolines are a great form of aerobic exercise, and any aerobic exercise will burn calories. Bouncing on a trampoline can also help with lymph drainage and is pretty safe on the joints.”

Aida Bauyrzhanova is a Kazakh artistic gymnast. She is the 2021 Islamic Solidarity Games champion on floor exercise. Born in Astana, Kazakhstan. She began training in gymnastics in 2005.
Bauyrzhanova made her international debut at the 2012 Junior Asian Championships where she helped Kazakhstan finish fourth as a team. Individually she placed fourth in the all-around. She turned senior in 2013. She competed at the Anadia Challenge Cup and the 2013 Voronin Cup. At the 2014 Asian Games she finished eleventh in the all-around.
At the 2015 Asian Championships she helped Kazakhstan finish fifth as a team and individually she finished thirteenth in the all-around. Later that year she competed at her first World Championships. She was unable to compete in international competitions in 2016 or 2017 due to an elbow injury.
She returned to international competition at the 2018 Asian Games. She finished twenty-third in the all-around. She next competed at the World Championships where she finished 111th during qualifications.
She competed at the Melbourne World Cup where she finished seventh on floor exercise. At the 2019 Asian Championships she finished nineteenth in the all-around. In December she competed at the Voronin Cup where she placed sixth in the all-around, fourth on uneven bars, third on balance beam, and second on floor exercise. She competed at the Koper Challenge Cup in 2021 where she finished sixth on floor exercise.

She competed at the 2022 World Cups in Doha, Cairo, and Baku. At the Asian Championships she finished ninth in the all-around and qualified to compete as an individual at the 2022 World Championships. Additionally she placed eighth during the balance beam final.
In August she competed at the Islamic Solidarity Games alongside Ayazhan Shamshitdinova. They finished third as a team behind Turkey and Uzbekistan. Individually Bauyrzhanova placed first on floor exercise, second on balance beam behind Dildora Aripova, and third on uneven bars behind Sevgi Seda Kayışoğlu and Bengisu Yıldız.

Women Fitness President Ms. Namita Nayyar catches up with Aida Bauyrzhanova an exceptionally talented Kazakh artistic gymnast, winner of 2024 Asian Championships Bronze medalist in Floor Exercises, here she talks about her fitness routine, her diet, and her success story.
You were born in Astana, Kazakhstan. Where did you have your early education? You made your international debut at the 2012 Junior Asian Championships where you helped Kazakhstan finish fourth as a team. This later propelled your career to the height where you leading Kazakh female artistic gymnast. Tell us more about your professional journey of exceptional hard work, tenacity, and endurance?
I have been a gymnast for quite some time, I am 27 years old at the moment. And I have been in sports for long 22 years! A career path of an athlete is definitely a hard one. Injuries didn’t always let me realize my potential to its fullest but I am delighted to admit my career has been so bright and I definitely had some meaningful victories!

Monica Austin-Cox remembers well the day a routine pelvic exam changed her life forever.
Her gynecologist felt a lump that was initially believed to be related to Austin-Cox’s ovaries. But further testing revealed it was actually a mass in her bladder.
She was quickly referred to a urologist, who performed a cystoscopy, a procedure that involves examining the inside of the bladder with a camera. The results confirmed her worst fears — Austin-Cox had bladder cancer.
The news was shocking: “I had never heard of bladder cancer,” said Austin-Cox, who was 30 at the time of her diagnosis and had never been a cigarette smoker but had been exposed to secondhand smoke much of her life. “The signs and symptoms, like blood in the urine, were things I had experienced, but I had attributed it to the repeated urinary tract infections (UTIs) I’d been getting. I’d always taken the antibiotics prescribed by my doctor and they would just go away. So, I thought nothing much of it.”

Her cancer was quickly diagnosed as stage 1, non-muscle invasive, an aggressive form of cancer that required close monitoring and repeated treatments. Over the next four years, Austin-Cox found herself in a nightmarish medical battle that included having surgery to remove the mass in her bladder and follow-up cystoscopies every three months. Each time, she’d learn that the cancer had returned.
“There was constant anxiety of wondering if the cancer would come back,” she recalled of the all-too-brief periods in between her checkups. “It was overwhelming.”
Adding to Austin-Cox’s anguish, she found little in terms of support groups and resources available for women battling bladder cancer, especially for Black women like her, near where she lived in North Chesterfield, Virginia. And she wasn’t able to find much information on the internet about how women experience the disease either. “It’s not just ‘a white man’s disease,’” she said, referring to a widespread myth. “Black women get bladder cancer too and we deserve the support and resources we need to fight this horrible disease.”
Men are more likely to develop bladder cancer than women and white people are about twice as likely to develop bladder cancer as Black and Hispanic people. However, a growing body of research confirms that Black women are battling the disease more and more, and they are often being diagnosed at later stages, facing poorer outcomes as a result.
Studies have also found:
Houston researcher Heather Honoré Goltz, Ph.D., an expert in cancer survivorship and disparities, a licensed clinical social worker, and a professor of social work at the University of Houston-Downtown, said women are often misdiagnosed, partly due to symptoms like frequent urination or blood in the urine being mistaken for menopause or UTIs, like Austin-Cox experienced. According to Goltz, Black women often face elevated risks for the disease related to exposure to harmful chemicals in certain professions, such as exposure to hair dyes while working as a hairdresser, environmental toxins where they live and the long-term effects of smoking, a leading cause of bladder cancer.
“Like your liver, the job of your kidneys is to filter harmful toxins from your bloodstream and move them into your bladder,” explained Goltz. “That’s why being exposed to certain chemicals may increase a person’s risk for bladder cancer.”
She attributes the poorer outcomes Black women with bladder cancer face to long-standing issues of bias within the healthcare industry. “A significant problem is the way healthcare systems interact with women, particularly Black women,” said Goltz, noting that studies show that even with health insurance, private and public, Black patients tend to receive lower quality of care compared to their white counterparts.
“There’s an assumption that if you have access to quality care, that you’re going to receive the gold standard of care, but that’s not always true. What we’re finding is that a large percentage of Black patients, and particularly Black women, aren’t getting the highest standard of care.”

Self-advocacy is vital in detecting and diagnosing bladder cancer in the earlier stages, when it’s more treatable.
“Be vigilant about any changes you’re experiencing in your body and any concerning symptoms, such as painful urination or frequent urination,” Goltz said. “Don’t automatically dismiss it as related to aging or menopause. Discuss your symptoms with your primary care doctor and request follow-up testing or even a referral to a urologist to be sure.”
Austin-Cox can relate to the quality of care concerns Goltz raised. Early in her diagnosis, she felt dismissed and neglected by her urologist, including receiving repeated requests from his office to reschedule follow-up exams, despite the severity of her case.
“The nurse would call and say, ‘He’s not going to be able to see you this week, would you like to reschedule?’” And I was like, ‘No, because the cancer keeps coming back,’” she recalled.
Frustrated, she eventually took control of her healthcare and sought a new urologist who provided more attentive care and initiated a more aggressive treatment, marking a turning point in her treatment. “Never be afraid to advocate for yourself,” she said.
Five years after her diagnosis, Austin-Cox was declared cancer-free. Now she visits with her urologist annually to check for any signs of the disease.
As for the lack of support and resources available for women battling bladder cancer, Austin-Cox, now 50, said that has improved slightly in the two decades since her diagnosis, but there’s still a great need today. She is grateful for a supportive husband, family and friends who helped her through her cancer journey. To this day, she pays it forward to others by volunteering and participating in advocacy efforts with the Bethesda, Maryland–based, Bladder Cancer Advocacy Network (BCAN).
Added Austin-Cox, “We all have to do our part to raise awareness about the fact that women do get bladder cancer — and our lives matter too.”
